Access Community Health Network is a Chicago-based health care organization that delivers preventive and primary care through a network of federally-qualified health centers, one of the largest such networks in the nation. It acts as the medical home for more than 150,000 patients annually in the Chicagoland area. The network focuses on patient-centered, accessible care to meet preventive and primary health needs in the community. In 2020, Access launched a program to provide free prosthetic devices, with plans to extend the service to children and young adults. In 2017, it piloted a virtual, integrated health home program for residents of Chicago's South and West sides with serious mental illness to improve outcomes.
