Adirondack Health Institute is an independent nonprofit organization in the health care sector that supports hospitals, physician practices, behavioral health providers, community-based organizations, patients, and other regional stakeholders. Its work centers on coordinating and facilitating efforts to transform care delivery and improve population health across the Adirondack region. Based in Glens Falls, New York, the institute acts as a collaborative partner within the regional health system, aligning clinical and community resources to address local health needs. In the region, the institute works with a network of health and community partners to expand access to evidence-based chronic disease self-management programs through the development of the Chronic Disease Care Coordination Network, in collaboration with the North Country Healthy Heart Network. It has partnered with Hudson Headwaters Health Network to situate collaborative efforts near Moses Ludington Hospital, reinforcing its role in regional health coordination. In August 2024, Adirondack Health Institute announced the appointment of Christopher Hickey to its Board of Directors, marking a governance development and continued engagement with regional stakeholders.
