Our South Florida Care Managers (Care Advocates) provide professional support and care coordination, especially important when family lives at a distance.  Care Managers are credentialed specialists with knowledge of geriatric physicians, professionals, and community resources.  Often retained by "adult children," Care Managers are proactive advisors for their parents’ health & well-being.

Care Management -  When medical issues are a key concern, or your loved one is in a hospital or rehabilitation center, experienced Nurse Care Managers are your “medical eyes and ears” and on-call 24/7.  Almost every senior states that their wish is to remain in their own home as long as possible. 

Home Care Management - Care at home by Certified Nursing Assistant or Home Health Aide caregivers with a light support layer of a Professional Care Manager (GCM) at no additional charge.  A Care Manager will help determine your needs and assist you to screen, interview, and select the best caregiver.  With multiple licensed agencies as providers the consumer has a wider selection, higher quality, competitive pricing, and more value.  

Transition & Placement - You know it’s time for a change. You’ve checked out several independent living or assisted living communities online, read website reviews, but now you need real life answers and viable options from a trusted local resource.  For the past 14 years, our Care Managers have worked with hundreds of clients living in senior communities, so we know these communities from the inside. Life Care Planning empowers seniors with a comprehensive, proactive plan to deal with challenges of aging, disability, and chronic illness.
