AIRnyc is a nonprofit organization based in the Bronx, New York, that delivers community-based health support through trained Community Health Workers. The program places CHWs in people’s neighborhoods to improve health, connect families with essential resources, and promote health equity, combining care coordination with coaching to guide individuals toward healthier paths. CHWs collaborate with clinical care management teams to assist patients living with chronic conditions such as asthma, COPD, congestive heart failure, and diabetes, or to provide maternity care support. In addition to direct medical guidance, AIRnyc disseminates public health information and conducts social needs screening and navigation to help clients facing housing instability, unemployment, and food insecurity. Having served vulnerable New Yorkers for more than two decades, the organization operates with a small-to-mid-sized staff and maintains partnerships with healthcare providers to pursue improved health and social care outcomes.
