All Care Health Center (ACHC) is a federally qualified health center (FQHC) located in Council Bluffs, Iowa. ACHC is certified as a Level III Patient Centered Medical Home (PCMH) by the National Committee for Quality Assurance (NCQA.) All Care Health Center opened in 1997 as the Council Bluffs Community Health Center and has experienced significant growth and expansion since its inception. ACHC’s mission is to provide affordable, comprehensive, high quality healthcare to the entire community.

ACHC serves three counties in southwestern Iowa and welcomes patients from Nebraska as well. ACHC is a one stop shop for patient centered care focusing on the underserved in our community and engaging our patients to live healthier lives. ACHC’s medical and dental services are supported with a comprehensive assortment of ancillary and access services including an onsite pharmacy, mental health providers, social services and health insurance enrollment assistance.
