The Anticoagulation Forum is a nonprofit organization focused on improving anticoagulation care by educating healthcare professionals and advocating for evidence-based best practices in antithrombotic medications. Founded in 1991 and based in Newton, Massachusetts, it is the largest organization of anticoagulant service providers, bringing together physicians, pharmacists, and nurses across thousands of practices. Its flagship Anticoagulation Centers of Excellence program provides access to tools, guidelines, and educational resources to help clinicians apply evidence-based best practices in patient care. The forum serves a broad professional community of about fifteen thousand clinicians across more than three thousand anticoagulation practices and supports over one million patients annually.
