BUFFALO PRENATAL - PERINATAL NETWORK, INC. is a company based out of 625 DELAWARE AVENUE, BUFFALO, New York, United States.We work with pregnant and parenting women at high risk for infant mortality and low birthweight infants in their homes. Our 2 women’s programs include Healthy Families NY ( part of Healthy Families USA)which focuses on parenting , child development , prevention of child abuse and neglect and encouraging self sufficiency in the mothers . The program follows the family until the child is 5 and this program is the largest program-in NY with a caseload of 400 families.Our other 
women’s program is our Community Health Worker Program. This program works with women from 14-40 through the life continuum. They strive to find women not connected to the system . This includes through street outreach, health fairs , etc. They provide intensive case management to their clients beginning with ensuring clients have health insurance and a health provider. They then work through all the other needs of the client including housing, food, clothing , baby basics, etc. 
We also operate a Nurturing Fathers program which provides support and education for fathers. We offer support groups at our agency focused on breastfeeding, education and empowerment ( for clients with depression) Outcomes for our program include over 90% in metrics including initiation into prenatal care, immunizations for children , attendance at well baby and postpartum visits , and lead screening. We serve approximately 1000 people yearly
