Care Dimensions provides hospice and palliative care across Massachusetts, integrating medical treatment with emotional, spiritual, and family support to help patients live as fully as possible. As the largest hospice and palliative care provider for both adults and children in the state, it serves more than 100 communities and delivers care across homes, skilled nursing facilities, assisted living communities, hospitals, and through two inpatient facilities: the Care Dimensions Hospice House in Lincoln and the Kaplan Family Hospice House in Danvers. The organization operates as a nonprofit, community-based provider of advanced illness care, offering hospice, palliative care, and grief support throughout Massachusetts. Its HomeMD program delivers in-home primary care to seniors over 65 in select communities north of Boston who have difficulty leaving home. Founded in 1978 as Hospice of the North Shore and now operating under the Care Dimensions name, the nonprofit also runs the Learning Institute, which educates more than 7,000 health care professionals and community members each year on advanced illness and end-of-life topics.
