Care Resources PACE is a Program of All-inclusive Care for the Elderly that provides wrap-around services to help adults aged 55 and older live independently in their homes. Operating through day centers in Grand Rapids and Lowell, the organization coordinates primary health care, home health services, rehabilitation, prescription medications, nutrition, social services, transportation, and other supports as part of a comprehensive care plan tailored to each participant. By drawing on Medicare and Medicaid funding to cover medically necessary care with little to no out-of-pocket costs for many participants, Care Resources targets residents of Kent County and specific nearby counties in Michigan, seeking to keep eligible seniors out of nursing homes through integrated, community-based care. Based in Grand Rapids and with a regional service area, the program focuses on delivering coordinated, multidisciplinary services that address physical, mental, emotional, and social needs to support ongoing independence for older adults. Recent developments include organizational leadership recognition in health care and ongoing expansion efforts in Lowell to increase capacity for more participants and staff.
