Community & Home Supports, Inc. (CHS) is a Detroit-based nonprofit established in 2008 to address homelessness. The organization uses a Housing First framework and provides a coordinated suite of services, including assessment, case management, financial assistance, wraparound supports, and data collection and advocacy, to help people secure stable housing. CHS participates in the Detroit Continuum of Care, serving in the roles of Coordinated Entry and Permanent Housing to coordinate access to housing resources. Through its Coordinated Entry program, CHS operates the Coordinated Assessment Model (CAM), which acts as the entry point for individuals in Detroit, Hamtramck, and Highland Park. Resource navigators assess housing crises, assist with essential documents, connect clients to services, and refer them to permanent housing providers within the CoC. In its Permanent Housing work, CHS assigns case managers to work with homeless households across Southeast Michigan, guiding them through initial assessments, individualized service plans, housing searches and landlord negotiations, ensuring ongoing financial support and wraparound services tailored to each household.
