Community Care Physicians (CCP) is the largest independent multispecialty medical group in the Capital Region of NY. Headquartered in Latham, we provide a broad range of healthcare services to 8 counties in upstate NY. Our organization includes primary care, specialty care, urgent care and wellness services. With more than 1,900 employees, including over 421 practitioners in its sub-specialty and primary care network, CCP touches every three to four people in the Capital Region.
