Community Crossroads is a New Hampshire nonprofit in the individual and family services sector that provides services to families and individuals with disabilities or chronic health conditions through conflict-free case management. It operates as one of ten state-designated area agencies, supporting people with developmental disabilities, their families, and adults with acquired brain disorders from birth through elder years, across eleven towns in southern New Hampshire. Founded in 1979 as Region 10 Client Management, the organization is based in Atkinson and serves more than 1,200 individuals and families, and offers an Oral Health Program accessible to those served. Since 2012 it has expanded to long-term care support under the Choices for Independence program and offers Representative Payee services for financial management, available to residents anywhere in New Hampshire. In 2017 it gained a Partners in Health contract to serve children with chronic health conditions, and in 2022 it partnered with Franklin Pierce University to develop leadership training for staff.
