Community Health Association of Mountain/Plains States (CHAMPS) is a nonprofit membership association of community, migrant, and homeless health centers in Colorado, Montana, North Dakota, South Dakota, Utah, and Wyoming (Region VIII). CHAMPS was founded in 1985 to unite Region VIII health centers in a mutual support network of community-based providers of primary care and related services for underserved communities in the mountain and plains states. This project is supported by the Health Resources and Services Administration (HRSA) of the U.S. Department of Health and Human Services (HHS) under cooperative agreement number U58CS06861, βState and Regional Primary Care Associations,β total award of $751,395 with 26% of the total PCA project financed with non-governmental sources.
