Community Health Network is a nonprofit IT services and consulting organization that supports community health centers by pooling business services to improve efficiency and reduce costs in delivering care to uninsured and underserved Tennesseans. It serves as a membership network comprising 17 health center organizations and three affiliates, collectively operating more than 100 clinics in Tennessee and affiliated with the Tennessee Primary Care Association. The organization is based in Savannah, Tennessee. 

CHN's early focus was information technology, including a statewide wide-area network and a shared practice management system. It has since broadened its offerings to include telehealth, a shared electronic health record, ePrescribing, and several joint purchasing programs to support its members. Through these initiatives, CHN aims to help member centers operate more efficiently and extend access to care for underserved populations.
