Community Health Partnership (CHP) was a small nonprofit in the hospitals and health care sector that served as a contracted Medicare and Medicaid care management organization. It integrated health and long-term support services and was based in Eau Claire, Wisconsin. The organization ceased operations on December 31, 2012. CHP has a noted history of collaboration in Colorado, including the 2017 deployment of ClientTrack, a care coordination platform for the Community Care of Central Colorado. In January 2023 the Colorado Healing Fund announced a partnership with CHP to create a navigator role for Club Q victims and to form an advisory committee to guide relief efforts.
