Community Hospice of Northeast Florida provides palliative and hospice care, along with related programs to support patients and families facing advanced illness. Founded in 1979, the organization serves communities across Baker, Clay, Duval, Nassau and St. Johns counties in Florida and has expanded its service area to 11 counties in North Central Florida.
