Community Physical Therapy & Associates (CPT) provides rehabilitation services in inpatient and outpatient settings to individuals in the greater Chicago metropolitan area and Wisconsin. Our board-certified therapists offer Physical, Occupational and Speech Therapies in post-acute rehabilitation and skilled nursing facilities, in the home setting and at outpatient locations.
