CBC is a New York city-based nonprofit Independent Provider Association (IPA) and Health Home serving more that 150,000 Medicaid-eligible New Yorkers with complex behavioral health and social-care needs. Our network of 70+ community-based agencies partners with hospitals, payors, city and state agencies, and people with lived experience to build a coordinated, person-centered system of care. What We Do -Care coordination at scale: CBC operates one of NYC's largest Health Home programs, delivering integrated medical, behavioral health, and social-care management for adults and children. -Innovative Programs: Flagship models like Pathway Home (hospital-to-community transition), Safe Options Support (SOS) (cross-sector outreach for people experiencing street homelessness), and Housing Location and Placement Services (HLPS) with NYC Health + Hospitals demonstrate measureable impacts in reducing avoidable hospital use, promoting housing stability, and improving quality of life. -Technology-enabled Hub: CBC's Hub provides centralized intake, data-sharing, dashboards, predictive analytics, and AI-enhanced tools to guide real-time decisions for care teams and payors. -Partnership for Systems Change: we collaborate with hospitals, payors, government, and philanthropy to align incentives, improve transitions of care, and advance health equity for vulnerable populations.
