Founded in 1968 to meet the community's needs, the Hill Health Center (HHC) emerged from a collaboration between the Hill Parents Association and Yale School of Medicine. As Connecticut's first Community Health Center, HHC has served New Haven neighborhoods for over fifty-five years. Originally focused on pediatric services for the Hill community, HHC recognized the increasing need for comprehensive healthcare solutions and expanded into other neighborhoods. The mission that remains today is to measurably improve the health and well-being of the community by providing excellent and compassionate care accessible to all. Today, as a Federally Qualified Community Health Center (FQCHC), CS-HHC has grown to serve more than 55,000 patients annually. CS-HHC takes pride in being a Patient-Centered Medical Home (PCMH) with a coordinated care system that creates a strong partnership between the patients and their Providers. CS-HHC continues its mission to provide equitable access to care with programs such as our Sliding Fee Scale (SFS) which allows patients who are underinsured or uninsured to receive care at a discounted service fee. With over 700 dedicated employees and 35 care sites, including 21 School-Based Health Centers, CS-HHC has locations in New Haven, Hamden, West Haven and Ansonia. While primarily in Greater New Haven and Lower Naugatuck Valley, CS-HHC proudly serves patients residing in 143 of Connecticut’s 169 cities and towns.
