Fair Haven Community Health Care is a nonprofit primary care provider based in New Haven, Connecticut. It was established in 1971 by community members and volunteers who sought to ensure health care was available to all, regardless of income. The center offers integrated services across medical care, dental care, behavioral health, and pharmacy, and serves the New Haven, Branford, and East Haven communities. Annually, the organization cares for more than 36,000 patients with over 150,000 visits. Its main campus is located at 374 Grand Avenue in New Haven, where FHCHC provides a coordinated primary care experience. The organization has earned recognitions for quality and inclusive care, including the HRSA Health Center Quality Leader designation for multiple years and the LGBTQ+ Healthcare Equality Builder award from the Human Rights Campaign Foundation. In 2025 it was honored with Best in Class: Healthcare at the CREW CT Blue Ribbon Awards for its facility, and it collaborates with local authorities to host community health events such as health fairs.
