HEALO 5 Mission The HEALO 5 mission is to create and facilitate a baseline of continued and evolving synchronicity between patients and their providers where everyone becomes smarter, more informed, and more consistent individually and together to provide the best possible care. Changing the communication channel where the patient can be, and is, open and honest in sharing information with the provider who then immediately provides education, advice, and direction to the patient based on their responses to the assessments drives unequaled participation in addressing how the patients are managing their pain. HEALO 5 Program Solutions The HEALO 5 Programs are a revenue positive Multi-Assessment approach consisting of three initial disciplines. Opioid Educational Assessment Program (OEAP) for providers/facilities who prescribe opioids to their patients with chronic or acute pain conditions. The program has been designed to change each month in order to have continued educational assessments of patients’ knowledge, understanding, and responsibilities as patients when they are prescribed and take their medications. Health Behavior Assessment Program (HBAP) for providers/facilities wishing to provide assessments and interventions to improve the patient’s health and wellbeing utilizing psychological and/or psychosocial procedures designed to ameliorate specific disease-related problems. Balance Assessment Program (BalAP) for providers/facilities utilizing a fall prevention balance and dizziness survey and subsequently using a balance plate to evaluate the patient. Acute Pain Program (ACP) is designed to specifically incorporate the framework of the baseline chronic program as well as additional OEAP items for those patients who have had an injury or other medical issue which may not require a long recovery period, yet may require opioids to combat the pain.
