The Health Foundation for Western and Central New York is a regional private foundation focused on improving health outcomes by advancing racial and socioeconomic health equity across western and central New York. Based in Buffalo with a second office in Syracuse, it serves eight western New York counties—Allegany, Cattaraugus, Chautauqua, Erie, Genesee, Niagara, Orleans, Wyoming—as well as central New York counties Cayuga, Cortland, Herkimer, Madison, Oneida, Onondaga, Oswego, and Tompkins. The foundation upholds the view that health care is a human right and aligns its activities with this principle. 

Notable initiatives include the Transform Rural Health campaign launched in 2023 to raise awareness of rural health disparities and advocate for systemic change. Its work includes funding programs and projects aimed at reducing disparities across its service area.
