Healthy Capital District is a nonprofit organization based in Albany, New York, founded in 1998 to improve public health and expand healthcare access for underserved children and adults in the Capital Region. It is designated by New York State to provide insurance navigation and enrollment services, reaching about 11,000 individuals each year. The organization also conducts social determinants of health screenings with referrals and operates the Seal a Smile program, providing free oral preventive care to more than 4,000 students annually. As the regional health planning organization, HCD supports the development of Community Health Needs Assessments and Community Health Improvement Plans across seven counties in the Capital Region. It develops the regional CHNA every three years and produced the regional CHNA for DSRIP, which was the highest-scored assessment in the state. In 2023, HCD launched a Certified Peer Development Program for CRPAs, and in 2025 it introduced an online Professional Development Platform offering continuing education credits.
