Healthy Plus Annual Wellness Visit- Healthy Plus provides a full service solution from scheduling to software, that increases the number of Medicare patient visits with minimal change of the current day-to-day workflow.
Chronic Care Management- Healthy Plus’s CCM program will make it easy for patient improvement and practice growth with no upfront cost or change to your workflow.
Patient Engagement Center- Healthy Plus’s patient engagement center provides additional support to hospitals, medical groups and privately owned physicians to enhance communication gaps and preventative care engagement between patients and their care provider. Our engagement center provides maximum scheduling output for increased preventative care visits.
