HHIO is a healthcare coordination company with our mission and primary focus being patient advocacy. We offer a new approach to the coordination of hi-tech home infusions, specialty injectables and pharmaceutical services.

HHIO consults and contracts with multiple providers, enabling us to provide seamless service for every patient’s home coordination requirements, discharging needs, or any health related clinical service within the home, regardless of demographics or insurance. HHIO assists our patients and their clinical healthcare team with a unique variety of coordination support needed to effectively manage a changing healthcare environment.

Every HHIO patient is assigned a clinical patient advocate to ensure that communication and support between all clinical teams is seamless within the continuum of care, starting with the ordering physician and facility and continuing with the primary care physician (PCP) and their clinical team.

Our patients are assigned a dedicated patient advocate to provide support for all initial and ongoing services, to the desired or required degree, in an effort to meet the individual needs of each patient. Advocates manage orders and organize the appropriate clinical documentation needed for verifying and authorizing insurance coverage. They educate their patients on the plan of care, medication indications and possible risks.

HHIO customizes every individual care plan by coordinating specialty pharmacies and home healthcare agencies that provide a specialized focus within specific disease states.

With a deep commitment to helping others, HHIO is passionate about supporting families in their growth. We provide a collaborative team approach to all of our fertility patients through detailed planning and communication, by obtaining injectable medications, related education and medication administration, all covered in the comfort of their own home.

We take pride specializing within the solid organ transplant community. Working with a patient’s transplant team ensures that we can coordinate the plan of care, pre and post-surgery. Through detailed planning and communication, we have been able to get our patients’ infusions covered in the home environment, reducing their hospital length of stay.
