By working in partnership with hospitals, consultants, community providers and GPs to provide ‘out of hospital’ care we can prevent some people being admitted to hospital and enable others to come home more quickly. Our highly skilled multi-disciplinary nursing and therapeutic teams can safely provide holistic care in the patient’s home, seven days a week. We use remote patient monitoring technologies. These provide effective monitoring and data-driven learning for the well-being of patients and effectiveness of the health system.
