Hospice of Northwest Ohio is a non-profit, community-based provider delivering end-of-life and palliative care to patients and their families in Northwest Ohio and parts of Southeast Michigan. Based in Perrysburg, Ohio, the organization traces its roots to 1981 and serves the terminally ill through in-home visits, nursing facilities, and freestanding hospice centers. It is the Toledo area’s largest hospice provider, accredited by the Joint Commission and certified to participate in Medicare and Medicaid, with a policy of never turning someone away for lack of ability to pay. In 2015, the organization cared for more than 2,500 patients and families, reflecting its scope and community impact. The entity aims to address medical, emotional, and spiritual needs for end-stage illness and to support caregivers, within the broader healthcare field. In 2025, the organization announced expansion of its offices, including a new location at Fulton County Health Center.
