Ignite Health exists to improve the health of the communities we serve, by leading a “personalized” approach to Pop Health & Interoperability, with strategic national partners who believe in the power of working together.

We are initially focusing on improving the health of the Appalachian region, where Heart Disease, Diabetes and the Opioid Crisis, has greatly affected the health of the population and the economy of the region.

With a national network of local health coaches for local jobs and local economic impact, we utilize the latest Interoperability and point-of-care technology, to identify health risks for Heart Disease and Diabetes.

Through this “personalized” approach, we can focus on measuring health data that matters, and “funnel” high risk community members to local health providers, for the right continued care to improve health outcomes.

We believe in putting patients at the center of their own healthcare, and lean on our local Faith communities for trust and support.

We have also seen a trend where rural communities health risks are rising, and we want to put the same amount of focus into improving the health of our rural communities that are struggling, that we put into our urban areas.

We also acknowledge the fact that we have a serious problem with increasing healthcare costs in our nation.

We believe we must work together with all community stakeholders including providers, carriers, patients and elected officials to establish a "Culture of Health", to solve these problems and bend the trend of unsustainable healthcare costs our nation faces today.
