Founded in 2011, Innovative Integrated Health, Inc. (IIH) is a premier integrative healthcare provider focused on enabling frail, underserved, and multiethnic senior communities to enjoy an improved quality of life and to age at home with dignity by providing high quality health and human services.  Our services are delivered through the Program of All-Inclusive Care for the Elderly (PACE) model of care.  

PACE by IIH provides a comprehensive health care plan and wellness services for seniors throughout California. All PACE by IIH participants receive a coordinated and individualized care plan managed by our interdisciplinary team. Care plans include primary, preventative and acute medical care as well as social, recreational, nutritional, transportation, and personal care services all personalized for their unique needs. With the support of PACE by IIH services, our senior participants live healthier, happier lives at home as an alternative to nursing home care. 

A Plan to Help Seniors Remain Independent

PACE by IIH provides a caring network of medical and social services which promotes independence, dignity and pride, and enables participants to live at home in the community with the help of friends and family.

PACE by IIH serves seniors who meet the following criteria: 

*55 years or older
*Reside in a PACE by IIH designated service area
*Can live safely in the community with the help of PACE services
*Require a level of care equal to nursing home services
