Lower Cape Fear LifeCare is a nonprofit health care organization, based in Wilmington, North Carolina, delivering hospice, palliative, memory care, and grief-support services across southeastern North Carolina and northeastern South Carolina. It traces its roots to 1980 when it began as a hospice and has since expanded to emphasize comfort-focused care, symptom relief, and patient-centered decision-making. Services are provided in patients’ homes, in assisted living and nursing facilities, within hospitals, and at inpatient hospice centers in Wilmington, Bolivia, and Whiteville. The LifeCare Memory Partners division has been accepted into CMS' GUIDE program, becoming the only provider east of Raleigh, north to Virginia, and south to Charleston to earn this recognition. The organization reports serving more than 1,000 patients daily across the region. In January 2026, Lower Cape Fear LifeCare launched a free-standing palliative care clinic in Lumberton, signaling continued expansion of its services.
