Managing Care Solutions provides Primary Care Practices with physician-led, team-based Chronic Care Management (CCM). Our unique service model of People + Technology enables practices to quickly and easily implement a Medicare-compliant CCM program that will allow you to provide high quality care to your qualifying patients and increase revenue for your practice — without investing in new staff or office space.

As you may know, Medicare's CCM program allows practices to bill CMS for the extra attention and care coordination beyond office visits that you provide to your Medicare patients living with 2 or more chronic conditions. A key challenge of CCM implementation is having the staff time and resources to call each qualifying Medicare patient in your practice for the minimum 20 minutes per month to comply with CMS rules.

• We'll enable tracking and billing via CPT Code 99490 for work that you are already performing, via our technology partnership with Smartlink Mobile

• Our team will build upon those billable hours by providing the full 20 minutes of telephone-based care coordination to more of your qualifying patients
