Natick Visiting Nurse Association (Natick VNA), which serves more than 20 towns in MetroWest, is the area’s premier free-standing, not-for-profit home health care agency. Too often, health care at home is misunderstood as being just for older people. We do offer senior care services—but Natick VNA provides so much more. We understand that there is often emotional, physical and financial stress associated with needing healthcare at home. Whether you have planned in advance for health care at home or face a crisis and need an immediate solution, we initiate care at home within 24-48 hours of referral; we provide care 365 days a year. Our Care Transitions Coordinators can meet with you, your caregiver or discharge planner to ensure a smooth transition from the hospital or facility to your home. We provide skilled nursing care, rehabilitation therapy (physical therapy, occupational therapy and speech-language pathology), medical social work and, end of life and bereavement services. Our clinical care is personalized and tailored to meet your individualized needs, resulting in high levels of satisfaction. Natick VNA helps patients of all ages live with the highest degree of independence in their own homes, utilizing the agency’s state-of-the-art home telemonitoring technology, ComfortCare palliative care program, cold laser therapy, and other advanced services. Natick VNA is one of only two home health agencies in Massachusetts to be granted a Double 5-Star rating by Medicare, based on our outstanding clinical outcomes and level of patient satisfaction.
