Our work includes developing, testing, implementing, and maintaining standardized patient assessment instruments and related guidance for post-acute care settings—home OAI supports multiple Centers for Medicare & Medicaid Services (CMS) contracts across PAC settings, including supporting the Quality Reporting Program (QRP) contracts for home health, hospice, skilled nursing facilities (SNFs), inpatient rehabilitation facilities (IRFs), and long-term care hospitals (LTCHs).
