PMS traces its origins to the United Presbyterian Church's Medical Mission in the Southwest, launched in 1901, and became a New Mexico nonprofit in 1969 to continue providing healthcare to underserved communities. It delivers an integrated mix of primary care, dental, behavioral health, and developmental disability services, along with veterans programs and childhood education, aimed at comprehensive, life-span care. The organization operates across roughly 100 locations in New Mexico, serving many people living at or below poverty level in urban, rural, and frontier areas where access is challenged by geography. Its centers have earned Joint Commission accreditation and some are recognized as Patient-Centered Medical Homes, with additional accreditation from the National Association for the Education of Young Children. 

Headquartered in Santa Fe, the nonprofit maintains a broad network of community health centers throughout the state. In 2023, PMS formed a partnership with HELIOS to improve care coordination for its community health programs.
