The Primary Care Coalition is a public health organization based in Silver Spring, Maryland, founded in 1993 by physicians and health care officials who imagined a community with broad access to high-quality health services. It was formed to share responsibility for care across public and private sectors, creating a framework to turn that vision into reality. Today, PCC coordinates a system of care through a network that includes independent safety-net health centers, private practices, and pro-bono specialty providers, serving about 35,000 people who face cultural, linguistic, and socioeconomic barriers. The organization connects individuals to health services via client services specialists and care managers who help with program enrollment, care coordination, behavioral health support, and access to medications. It also provides technical assistance to safety-net clinics in areas like quality assurance, process improvement, and health informatics, while bringing together the local health department, hospitals, and other organizations to address shared challenges. A recent development is a May 2025 partnership with MCC Chicago to deepen data-driven service delivery.
