Rainbow Hospice and Palliative Care is a 501(c)(3) non-profit organization. A Medicare-certified hospice that is accredited by the Community Health Accreditation Program (CHAP), Rainbow is one of the oldest and largest non-profit hospice organizations serving patients and families in Chicago and the surrounding counties. Rainbow Hospice and Palliative Care teams provide physical, emotional, psychological and spiritual relief to patients and families living with advanced, complex illness. Our mission is to empower people to live with hope and resilience throughout the journey with illness and loss. Rainbow Hospice and Palliative Care is proud to partner with Presence Health. Our support continues beyond the trajectory of illness; we offer ongoing support for bereaved individuals through our Grief and Loss Services, as well as continuing education for healthcare professionals and members of the community through our LIFE Institute for Learning. Rainbow Hospice and Palliative Care is proud to serve nine counties in Illinois: Cook, DeKalb, DuPage, Kane, Kendall, Lake, McHenry, Will and parts of Boone.
