Rehab Care Coordination is a care management company with over 25 years of experience in providing life care planning and case management services to clients (children, adults & families) with traumatic brain, spinal cord, and orthopedic injuries as well as mental illness. We were the first company in San Diego to specialize in Case Management and Life Care Planning. Since then we've managed the care of hundreds of patients throughout California as well as written life care plans and testified in 12 states.
