SFCCC develops innovative programs and engages in policy advocacy to broaden access to high-quality, community-based primary health care. It strives to ensure care that is comprehensive, coordinated, and efficient, and that respects cultural and linguistic needs while prioritizing the patient experience. Working with providers citywide in San Francisco, SFCCC coordinates primary care with specialists, hospitals, and other services, reaching more than 10 percent of the city’s population and serving groups that are often underserved, including low-income, homeless, immigrant communities, as well as LGBTQ individuals, people living with HIV, and seniors. In December 2017, SFCCC partnered with St. Anthony Foundation to host a press event highlighting the challenges faced by medically underserved communities.
