The goal is to drive sustainable behavior change… to help individuals form new habits, make better choices, sustain healthier habits and manage chronic conditions Our CareCentra Intelligent Health Platform powered by MoBe, a proprietary set of behavioral mapping algorithms, segments individuals into 16 segments that differ in their susceptibility to a particular disease, their response to a specific treatment, or in their motivations and abilities to take therapeutic or preventative health actions. For each segment, MoBe deploys a series of condition-specific nudges matched to the behavioral propensities of that segment. Over time, using quantitative techniques and feedback from health outcomes, the platform personalizes nudges to individual patients within each segment to shape their health behavior.
