Senior Care Concepts works with seniors, spouses and families to assist their loved ones in maintaining their well-being, independence and dignity through the aging process.  Our focus as Comprehensive Care Managers is to assess clients’ current and future needs, coordinate care, provide ongoing assessments and modify the care plan as necessary. We are client and family advocates, ensuring all needs are met and follow the clients throughout the continuum of care.  As services are needed, such as home health care or a physician referral, we as care managers seek out providers that best suit the clients’ situations.  During a crisis, we are able to mobilize in a very timely manner-sometimes at a moment’s notice.  Should clients need to consider alternate living arrangements, we assist in this transitional process as well.  We meet directly with clients and their families wherever it is most convenient for them.
