South Eastern Melbourne Primary Health Network (SEMPHN) operates as a regional health planning organization within the hospitals and health care sector, focusing on a more equitable, person-centred and seamless health system for its communities. As one of six PHNs in Victoria and 31 across Australia, SEMPHN is funded by the Australian Government to address priority areas such as mental health, alcohol and other drugs, Aboriginal and Torres Strait Islander health, aged care, population health, health workforce development, and digital health. Its local focus emphasizes consumer-centred care, service demand management, innovation, system reform, and organizational sustainability within the South East Melbourne region, based in Heatherton, Victoria. In 2026 SEMPHN partnered with Monash University and the Quality Use of Medicines Alliance to promote education on the quality use of medicines for general practice.
