Tender Hearts Home Health Care provides an array of services that will enable their clients to live independently while maintaining a safe and healthy quality of life. Services range from Companionship Care to Personal Care, Alzheimer’s and Dementia Care and also, Hospice Support.

- Maintain a Safe and Healthy Environment for the Client
- Medication Reminders/care plans
- Cognitive Stimulation
- Follow up with PT/OT Exercises
- Monitor  Health Changes
- Provide Emotional & Physical Support  
- Written Log of Clients Day for Family
SKILLED CARE:
- Companionship Care Plus:

- Daily Hygiene:
- Toileting Needs
- Commode Assistance
- Incontinence Care  
- Changing Briefs
- Oral Hygiene
- Empty Catheter bags 
- Grooming and Assistance with Dressing
- Transfers: Wheelchair, Slide Board & Hoyer Lifts

- Monitor Vital Signs
- Alzheimer’s and Dementia Care: Trained & Certified
- Hospice Support: Working Together with Trained Hospice Nurses
- Assistance after Surgery or Injury
OVERNIGHT CARE:
- Companionship or Skilled Care Plus:
- A Caregiver to sleep over so that you or your loved one is not alone
- A Caregiver to assist getting ready for bed
- A Caregiver to assist with repositioning or toileting through the night
- A Caregiver for constant monitoring through the night
