Caring for an individual who needs assistance due to aging, dementia, disability or serious illness can be challenging. Don’t go it alone. A  Certified Geriatric Care Manager (GCM) will evaluate the senior's level of safety, independence, and comfort in their current living situation, and conduct an overall assessment of their status.  An individualized care plan will be created that includes recommendations, identification of resources and possible funding sources available to improve their situation and address a variety of concerns.   Follow-up services include ongoing care management, care coordination, reassessment, ongoing monitoring, crisis intervention, patient advocacy, caregiver support, and consumer education.
