OSJCT is a not-for-profit charitable trust that provides care, support, and housing for older people and individuals with learning disabilities. It expanded through a February 2024 merger with The Fremantle Trust, combining resources to deliver care more cohesively. In September 2025, services in Buckinghamshire were integrated into the Trust, broadening its coverage and adding learning disability services to the established older people2s care. The organization is based in Lincoln, England, and operates across multiple counties to serve a broad regional population.

It supports over 4,000 people and employs thousands of staff, building a network of 75 care homes, eight extra care housing schemes, and 16 learning disability services (five residential and eleven supported living). Its offerings cover dementia care, intermediate care, respite and day care, expert nursing, and dedicated services for people with learning disabilities. The group has been recognized for quality through accreditation by the National Care Forum under its Quality First framework, and it has historically been among the first to employ Admiral Nurses. The Trust's footprint includes Buckinghamshire, Gloucestershire, Lincolnshire, Oxfordshire, Suffolk, West Sussex, and Wiltshire, reflecting its continued growth and regional focus.
