We also help patients manage chronic disease with education, medication, nutrition, and counseling. 78% of our 22,000 patients lie at or below the poverty line, so we strive to offer affordable and accessible health care to the most vulnerable individuals in our community. By doing this, we believe we are able to narrow health disparities among population groups, reduce the effects of chronic disease, as well as prevent unnecessary and costly hospitalizations.
