VNA Foundation is a grantmaking foundation serving the Chicago metropolitan area. We provide financial support to nonprofit organizations that offer home- and community-based healthcare to the medically underserved.  

VNA Foundation awards grants in Cook, Lake, McHenry, DuPage, Kane and Will counties with a focus on Chicago. We award program, operating, and capital grants which are in support of the following areas:
(1) Home health care services, (2) Community and school-based services, (3) Primary care and chronic disease management, and (4) Health promotion.  In deference to our roots as the Visiting Nurse Association of Chicago, priority consideration is given to programs in which care is provided by nurses.
