AccessCare is a not-for-profit organization in the hospitals and health care sector, founded in 1998 as a member-owned coalition of 23 practices. It operates as a statewide network of primary care providers focused on expanding access to medical services, maintaining quality of care at no charge to clients, and reducing costs for the state. Within North Carolina's Medicaid program, it is the largest of fourteen local networks contracting with the state Department of Health and Human Services to develop disease management, case management, and prevention and treatment initiatives, coordinated by Community Care of North Carolina to connect enrollees with a medical home. As of 2011, the network included more than 300 primary care practices and around 1,000 providers, serving over 227,000 Medicaid enrollees. Based in North Carolina, AccessCare sits within the state's public health care framework, aligning physician-led partnerships with state care-coordination efforts under the Carolina ACCESS program. The arrangement emphasizes coordination of care to improve outcomes while containing costs by linking Medicaid enrollees with a consistent medical home.
