WHO WE SERVE
We provide in-home skilled services inFairfield County and portions of western Perry County for individuals who need assistance to live independently. Among those we serve are …
• People with heart and lung disease, diabetes, cancer and other chronic illnesses;
• Patients recovering from illness, injury after a fall, or surgery; and
• Patients recently discharged from a hospital, nursing home or rehab center.
GETTING STARTED
Our services may be requested directly by you or a family member. We coordinate the services with your physician. You do not need to be discharged from a hospital to qualify for our services.
FIRST VISIT
Usually the nurse visits the patient within 24 hours of the referral, often the same day if returning home from a hospital or facility stay. Therapists almost always begin treatment within 2 days.
MEDICARE AND INSURANCE
Medicare pays the full approved costs of covered visits. Veterans are also eligible for services. Call our Nurses at 740-653-2224 with questions about your benefits if you have private insurance.
