Founded in 1999, ACMA is the only professional membership organization created specifically for Hospital/Health System Case Managers. We provide quality resources, solutions and support for the practice of Case Management and Transitions of Care. Our national membership is composed of a variety of professionals including nurses, social workers and physicians, among others. 

Our mission is to be THE Association for Health Care Delivery System Case Management and Transitions of Care (TOC) Professionals. We aim to accomplish this mission by providing innovative professional development services and networking opportunities for our members, as well as by influencing the policies and laws related to the practice of case management.
