Choice in Aging is a nonprofit organization operating in the hospitals and health care sector, based in Pleasant Hill, California. Since 1949, it has partnered with families and caregivers to deliver individualized assessments and care plans for adults, with a focus on individuals who have multiple chronic conditions, including mid to late stage Alzheimer’s disease and dementia. The organization runs three programs: two Adult Day Health Care centers in Contra Costa County that provide in-community services with staffing similar to skilled nursing, and two home- and community-based programs designed to support aging in place. The Multipurpose Senior Service Program offers Medi-Cal–funded case management for seniors at a skilled nursing level of care, coordinating medical and social support to help clients remain independent. California Community Transitions, part of the federal Medicaid program, assists former long-term institutional residents in transitioning back to community living through housing, services, and safety coordination. The organization serves adults and their families, addressing care coordination, in-home support, and transitional services to improve quality of life and independence.
